LPN Clinical Interviewer

Do you enjoy using your clinical knowledge to help people better understand their medications - but prefer a structured, telephonic environment over traditional bedside care?


As an LPN Clinical Interviewer, you'll speak directly with members to understand how they are taking their medications, identify barriers to safe and effective medication use, and help connect them with the right clinical support. Your conversations can make a meaningful difference in medication adherence, safety, and health outcomes—particularly for members managing complex health needs.


This is a high-volume, structured clinical role. You'll work within established workflows, use multiple systems, and be accountable for defined productivity, quality, and documentation standards. What You'll Do


Talk with members about their medication


  • Conduct telephonic medication interviews and collect information needed to support Medication Therapy Management (MTM) activities.


  • Verify member identity and obtain appropriate consent before discussing medication-related information.


  • Review and reconcile prescription, OTC, herbal, and supplement medications.


  • Ask questions to understand medication adherence, effectiveness, side effects, affordability, and barriers to proper use.


  • Identify potential medication-related concerns and document findings accurately.


Educate, support, and escalate when needed


  • Provide medication education in clear, easy-to-understand language within your scope of practice.


  • Reinforce appropriate medication administration, dosing, storage, and adherence strategies.


  • Recognize urgent, complex, or out-of-scope concerns and escalate them appropriately.


  • Partner with pharmacists, care coordinators, and operational teams to support appropriate follow-up.


Document accurately and meet quality standards


  • Document member interactions, assessments, and outcomes in designated systems.


  • Follow established clinical workflows, scripts, decision trees, and escalation pathways.


  • Maintain documentation that meets clinical, regulatory, and audit standards.


  • Meet established productivity, quality, and documentation expectations.


  • Participate in quality reviews, coaching, audits, and other performance activities.


Work efficiently in a high-volume environment


  • Navigate multiple systems simultaneously, including the MTM platform, call systems, email, and documentation tools.


  • Manage outreach, documentation, and follow-up activities efficiently.


  • Stay current on commonly used medications and program updates.


  • Maintain a strong focus on member experience, accuracy, and medication safety.



What We're Looking For


  • Graduate of an accredited LPN/LVN program.


  • Active LPN/LVN license in good standing.


  • Experience in a healthcare and/or telephonic outreach environment.


  • Comfortable working in a high-volume, metrics-driven environment.


  • Ability to navigate electronic documentation systems and Microsoft Office tools.


  • Experience with Medication Therapy Management (MTM), preferred.


  • Experience with Transition of Care, preferred.


  • Experience working with Medicare or Medicaid populations, preferred.

  • Put safety first: You understand that accurate documentation, appropriate escalation, and member safety matter more than simply moving quickly.



What We Offer


Location: Hybrid (3 days in office, 2 days from home) - 4645 E Cotton Center Blvd STE 100, Phoenix, AZ 85040


Salary: $28.00 per hour + Quarterly Performance Based Bonus| Additional $1 per hour for Bilingual (Spanish) speaking candidates!


Schedule: Monday to Friday 8:30AM - 5PM MST (Local Arizona Time)
