Our mission is to bring the highest quality of care to those individuals suffering from Opioid Use Disorder (OUD). We understand not only how difficult it can be to recover from addiction but that the road to recovery may be a long and bumpy one. And we know for sure that to achieve success, this road must be traveled with as much support as possible.
Care Manager is responsible for advocating for and assisting patients with every aspect of their physical and mental health by collaborating with various medical providers and internal team members.
Essential Duties and Responsibilities include the following and other duties that are assigned:
REQUIRED KNOWLEDGES, SKILLS, AND ABILITIES:
MINIMUM EXPERIENCE AND TRAINING NECESSARY:
A bachelor’s degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice, or other related social sciences with personal recovery experience
OR
An Associates degree in a social science field with any equivalent combination of experience and training with lived recovery experience.
Ability to interpret regulations, policy and procedures and apply them accordingly.
Ability to work with persons with physical, mental, or emotional disabilities or who are economically disadvantaged or who are involved with the criminal justice system.
Ability to establish and maintain effective working relationships with other members of staff, outside agencies and institutions and the public.
Pay: $18-$21 per hour
