Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. You are a Certified Home Health Aide in California who is interested in working with an interdisciplinary team of health professionals helping individuals that have transitioned from a hospital to home care setting and assisting them to remain in the community. You enjoy helping clients achieve their personal goals of remaining in the community. You want to work with a team that inspires professional growth and share our priority of providing quality case management and home health care to the community. Note* This is not a position for Caregivers, or Registry Aides. We are accepting applications from Certified Home Health Aides who have completed training as a Home Health Aide and received the necessary license issued from the State of California.

Service Region/ Location: This is not a remote role and requires you to work out of the Chico office seeing patients in Chico and Butte County and in surrounding counties.
Schedule : Part-time or Per-Diem work - Ranges from 2-29 hours per week, hours may vary. Strong possibility of full-time work in the next 90 to 180 days.
Travel : You will need to travel to client's homes within the Chico area and surrounding counties.
Compensation: $23.00 - $25.00 / hour.
Physical Requirements: Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirements for this role specifically include:
