We are innovative, creative, and continuously improving. Job Description: We are looking for Family Nurse Practitioners to provide primary care services as part of our primary care team. The FNP will work with a multi-disciplinary team to care for and provide medical services to patients, including examining/conducting tests; analyze records, reports, and test results; to prevent and/or diagnose illness, disease or injury; prescribe/administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, and/or injury; explain procedures and test results on prescribed treatment plan; manage labs, referrals. The FNP will collaborate with other physician assistants, nurse practitioners, physicians, and other support staff as part of the clinical team. Perform other job-related duties as required by departmental needs/demands. Experience and Skills: Completion of an approved/accredited educational program for Nurse Practitioners Valid NY State license, CDS, DEA, and BLS Ability to effectively communicate, follow established procedures and protocols Understanding of and passion for Community Healthcare is preferred Benefits: At Urban Health Plan, we value our team and foster a positive, fun work environment!
