Essential Functions Effectively manage a case load with co-occurring disorders. Conduct Comprehensive Assessments and Surveys for clients. Maintain regular communication with referents to provide progress updates and consultation for continuing care planning. Assess and develop Individual Abuse Prevention Plans for vulnerable clients as per Program Abuse Prevention Plan criteria. Create individualized treatment plans with specific goals and interventions for each client. Document client progress compliant with policies. Collaborate with the Recovery Services Manager and Mental Health Professionals on client treatment and progress. Conduct group counseling and education sessions. Assistclients in developing and implementing personalized continuing care plans, including coordinating with external services. Provide resources and support to family members and friends for sober living duringand post-treatment. Coordinate with the school system to address academic needs. Participate in professional development activities related to substance use disorders and Co-Occurring Disorders. Participate in community outreach as it applies to Recovery Services programmingKnowledge, Skills, and Abilities Licensure by the State of Minnesota for Alcohol and Drug Counselor in good standing OR Temporary Permit through the State of Minnesota in good standing. Must be able to pass DHS background study and meet applicable requirements and regulations. Three to five years' clinical experience preferred. Ability to perform basic health assessment procedures, CPR, first aid, and client care functions, or upon employment completion of training; Mustbe free from problematic substance use for at least two years immediately preceding their hiring and must sign a statement attesting to that fact. Excellent oral and written communication skills. Excellent computer and organizational skills. Microsoft Outlook/Office and Electronic Health Record experience required. Ability to structure time well, develop and meet priorities. Knowledge of legal and ethical issues related to patient's rights Ability to work in a team environment. Understanding of HIPPA and Mandated Reporting laws, MN State Statutes including Chapter 245G, Chapter 245A, and related sections. Additional duties may include working with patients in either the medical department and Medication Assisted Treatment (MAT) clinic, the behavioral health department or other departments within the Indian Health Board, Inc. In addition to competency in delivering evidence-based assessment and intervention, must also be willing and able to inform clinical practice with respect to the dynamics of underlying collective, generational and historical trauma as well as the spiritual and cultural tenants of the American Indian/Indigenous culture. Physical DemandsThe work requires some physical exertion such as long periods of standing, walking, recurring bending, crouching, stooping, stretching, reaching, or similar activities; recurring lifting more than 50 pounds. The work may require specific, but common, physical characteristics and abilities such as above-average agility and dexterity. The work environment involves high risks with exposure to potentially dangerous situations requiring a range of safety and other precautionsHIPAAIHB makes reasonable efforts to limit access to and use of protected health information (PHI) by employees to the minimum necessary performance of assigned duties as outlines in job descriptions. This position is access restricted to PHI needed to carry out health care operations.
