Social Services Director Skilled Nursing
Job Summary:
We are seeking a compassionate and experienced Social Services Director Skilled Nursing to join our team. As a critical member of our care team, you will be responsible for ensuring the highest level of emotional, social, and psychological well-being for our residents. Key Responsibilities:
• Develop and implement comprehensive social services plans to meet the unique needs of each resident
• Conduct comprehensive assessments and provide individualized care plans to promote resident independence and autonomy
• Coordinate and facilitate resident activities, including life-enrichment programs, outings, and social events
• Collaborate with interdisciplinary care teams to ensure holistic care and address resident concerns and needs
• Provide emotional support and counseling to residents, families, and caregivers
• Develop and maintain relationships with community organizations and service providers to enhance resident services
• Ensure compliance with state and federal regulations regarding social services
• Supervise and direct social services staff, including social workers, activity directors, and other support staff
Requirements:
