Location: Palm Beach County, Florida
Schedule: Full-Time | 8:00 a.m. – 5:00 p.m.
Salary: $75,000–$78,000 per year
Healthcare & Rehabilitation is seeking an experienced and compassionate Director of Social Services to lead our social services department and advocate for the well-being of our residents and their families. This is an excellent opportunity for a dynamic, resident-centered professional with strong leadership abilities, clinical insight, and regulatory expertise.

Develop, implement, and maintain departmental policies in compliance with federal and state regulations
Lead, mentor, and support the Social Services team
Advocate for resident rights and promote a culture of dignity, compassion, and respect
Participate in interdisciplinary care planning meetings
Coordinate and oversee discharge planning to ensure smooth resident transitions
Monitor and enhance the quality of social services to improve resident outcomes
Ensure accurate documentation and regulatory compliance
Prepare and present Quality Assurance (QA) reports
Serve as the facility’s Grievance Officer, addressing concerns with professionalism and empathy
Bachelor’s Degree in Social Work (BSW) or related human services field preferred
Minimum of 2 years of experience in a healthcare or long-term care setting
Strong knowledge of federal and state long-term care regulations
Excellent communication, leadership, and organizational skills
