Job Title: Physical Therapist Profession: Physical Therapist Specialty: Therapy Duration: 13 weeks Shift: 5x8.0 Hours per Shift: Monday - Friday 8am - 4:30pm Experience: Minimum 1 year of geriatric rehabilitation experience License: Client Physical Therapist license Certifications: None specified Must-Have: BS degree in Physical Therapy Description: Perform comprehensive evaluations within twenty-four hours of referral. Plan and implement an appropriate treatment program with measurable, objective, realistic goals based upon evaluation data. Complete necessary documents following evaluation, including contracture evaluation, balance sheet, physician certification for restorative treatment, and therapy attendance sheet. Consistently demonstrate safe resident care techniques. Maintain legible, complete records of resident evaluations, progress notes, and discharge notes in the computer system and resident charts. Work as an integral part of the rehabilitation team to meet resident needs. This includes collaboration with the departmental team as well as resident/family instruction and contact with involved health professionals. Perform resident treatments as outlined in evaluations, making appropriate changes as resident status changes. Treatments may include transfer training, bed mobility, ambulation, exercises, massage, and use of electrical or battery-operated modalities. Attend scheduled family and clinical meetings. Assist with the supervision and training of new personnel, assistants, aides, students, and volunteers. Attend continuing education courses/seminars as appropriate, with follow-up in-services to the department. Perform other duties as assigned. Ability to read, analyze, and interpret common journals, reports, and legal documents. Assist in the supervision of one Physical Therapist Assistant and one Physical Therapy Aide/Transporter. Must be able to apply basic mathematical concepts. Able to apply concepts such as percentages, fractions, ratios, and proportions to practical situations. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
