Position Summary: Thisposition works with clients to develop a care plan that will lead to stability,self-sufficiency, and create conditions amenable to client self-management oftheir health. Care coordinators willparticipate on the team of providers, and may be the lead of that team. Therole of the Care Coordinator includes assisting the client in accessing medicalprograms. This includes CAREAssist,Medicaid, and Medicare. Ongoing advocacy and support is provided to the clientin order to maintain access to medical assistance programs, and minimize anybreak in access to medical care. This position will assist clients in accessingprivate and government social services to meet basic needs such as housing,transportation and food, as well as alcohol and drug programs, mental healthservices, vocational programs and SSI/SSDI. In addition, this positioncoordinates access to support services provided through the Ryan WhiteTreatment Modernization Act.
Essential Functions and Responsibilities: - ClientManagement: Carry an active caseload of eligible clients, providing directone-on-one supportive case management services. Able to provide services in-person,over the phone and on a drop-in basis. Assist with coverage of services.
- Assessments& Planning: Conduct Ryan White intakes, comprehensive screenings, and goalplanning to identify psychosocial needs and barriers to care. Developindividualized service plans based on assessments and available resources.
- HealthInsurance Support: Assist clients in signing up for health insurance, includingMedicare, Medicaid and CAREAssist.
- Coordinationof Services: Work collaboratively with intake coordinators and medical casemanagers, providers, community partners to facilitate access to HIV care,social services, mental health services, and public benefits.
- Documentation:Maintain accurate and timely documentation of client interactions, placements,services provided, and progress updates in agency databases. Process andcollect necessary eligibility documents including Authorization for Release ofInformation.
- Crisis Intervention: Respond to psychosocial crises andfollow up.
- Referrals: Refer clients to local resources as needed andfollow up.
- Community Engagement: Build partnerships with localservice providers. Attend relevant community meetings and participate inongoing training and professional development.
- Client Events: Assist with planning and attending clientevents on a monthly to quarterly basis. This includes shopping for food,cooking, and socializing with clients at events. Ability to attend eventsoutside of normal business hours.
- Travel: Ability to travel to see clients in othercommunity settings and provide home visits on occasion. Ability to transportclients on occasion.
- Standards: Comply with all applicable program standards,guidelines, laws and administrative rules.
- Abilityto work well with others, handle stress appropriately, have reliable attendance
- Abilityto use personal cell phone to run agency phone system software, MFA, etc.
- Other tasks as assigned,including training/cross-training other staff as needed
Required Qualifications: - Bachelor'sdegree in social work/human services related field or 2 years full-time casemanagement experience, or equivalent combination of relevant and/or relatedexperience and education
- TBtest (provided at agency)
- ValidOregon driver's license, or valid out of state driver's license with the abilityto get an Oregon driver's license within 30 days of establishing residency inOregon, and a driving record sufficient to be covered by agency auto insurancepolicy.
- Abilityto pass agency criminal background check
- Abilityto obtain OHA Application Assister ID (cost covered by agency)
- Abilityto obtain Food Handler's Card (cost covered by agency)

Physical Demands: Whileperforming the duties of this job, the employee is required to
- Repetitivelyuse hands, including grasping, twisting/turning of wrists, and finger dexterityto perform various necessary clerical duties, such as typing and writing.
- Frequentlyuse arms, such as pushing & pulling to open/close drawers, cabinets, etc.
- Sitand/or stand for extended periods of time at an assigned station
- Bend,stand, & reach to be able to transport clients and to lift and carry up to25 pounds for tasks such as building kits & assembling testing/educationalmaterials.
- Use continuousclarity of vision at 20 inches or less for processing of paperwork
