Care Coordinator
The Care Coordinator plays a key role in a high-volume, multi-specialty medical clinic by managing patient intake, insurance verification, appointment scheduling, and ongoing patient communication. This position supports a diverse patient population across pediatrics, urgent care, adult care, and radiology, while working closely with physicians, pharmacies, laboratories, and other healthcare professionals. The role requires strong customer service, technology proficiency, and the ability to guide patients through both in-person and telehealth visits.
Responsibilities
- Perform patient intake, including checking in patients and accurately collecting and updating patient information.
- Complete insurance verifications to ensure coverage and eligibility prior to visits and procedures.
- Schedule a high volume of patient appointments, managing large numbers of incoming calls within a multi-person scheduling department.
- Schedule patients correctly based on the type of visit requested, ensuring the appropriate physician is assigned in accordance with approved scheduling guidelines.
- Use Jelly Bean or similar electronic refill request tools to process and route prescription refill requests from outside pharmacies.
- Respond promptly and professionally to patient portal messages, addressing questions, concerns, and requests.
- Key patient information into the electronic medical record system in a timely, accurate, and efficient manner.
- Make physician schedule changes as needed and generate monthly appointment schedules to support clinic operations.
- Coordinate with patients, providers, pharmacies, and laboratories to facilitate patient orders, prescriptions, and follow-up care.
- Maintain a high level of professionalism and strong customer service throughout the day in all patient and staff interactions.
- Demonstrate flexibility in job duties and functions, adapting to changing clinic needs and workflows.
- Communicate effectively with a geriatric patient community, using clear, respectful, and patient-focused language.
- Utilize strong technology and video conferencing skills to help facilitate and guide patients through telehealth appointments.
- Work within the EMR system (E Clinical Works) to document, update, and retrieve patient records and appointment information.
- Apply medical terminology, medication knowledge, and pharmacy knowledge when handling patient inquiries and prescription-related tasks.
- Fill and process prescription requests accurately, following established protocols and ensuring appropriate documentation.
- Exhibit strong phone etiquette and communication skills when interacting with patients, providers, pharmacies, and laboratories.
- Maintain detailed and organized records, ensuring all patient and appointment information is accurate and complete.
Essential Skills
- 12 years of recent medical office experience in a clinical or ambulatory care setting.
- Basic Life Support (BLS) certification and/or current CPR certification.
- Proficiency in medical terminology and its application in a clinical environment.
- Medication knowledge or pharmacy knowledge sufficient to support prescription requests and patient education.
- Experience working with electronic medical record (EMR) systems, preferably E Clinical Works.
- Strong computer skills, including the ability to navigate multiple systems and video conferencing platforms.
- Demonstrated ability to perform insurance verification and manage patient scheduling.
- Excellent phone etiquette with the ability to handle high call volumes professionally and efficiently.
- Strong communication skills for interacting with patients, providers, pharmacies, and laboratories, including geriatric patients.
- High attention to detail and accuracy in data entry, scheduling, and documentation.
- Ability to fill and process prescription requests in accordance with clinic protocols.
- Customer service mindset with the ability to maintain professionalism in a fast-paced environment.
Additional Skills & Qualifications
- Experience in medical assisting, urgent care, internal medicine, or high-volume clinical settings.
- Experience working with pediatric, adult, and urgent care patient populations.
- Familiarity with EKG procedures or urgent care workflows is beneficial.
- Strong patient service orientation and ability to work in a multi-specialty medical group.
- Comfort using telehealth and video conferencing tools to support remote patient visits.
- Ability to work collaboratively in a tight-knit, family-oriented team culture.

Work Environment
This role is based in a high-volume medical clinic that serves pediatrics, urgent care, adult care, and radiology patients. The Care Coordinator works Monday through Friday from 8:15 a.m. to 5:15 p.m. in a fast-paced environment that supports approximately 17 physicians and can see more than 250 patients per day. The clinic uses E Clinical Works as its EMR system and relies on phone, video conferencing, and telehealth technologies to deliver care. The culture is very family-oriented, with a tight-knit and welcoming team that values collaboration and mutual support. The location is within walking distance of several restaurants and amenities, offering convenient options for breaks and meals. Professional attire appropriate for a medical clinic is expected, and the environment emphasizes patient-centered service, efficiency, and teamwork.

This is a Contract to Hire position based out of Fountain Valley, CA.
Pay and Benefits
The pay range for this position is $22.00 - $24.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
