We're seeking the right candidate for the position of Social Worker at Chateau De Notre Dame Community Care Center. In this vital role, you'll ensure that our residents receive the highest level of care while collaborating with a dedicated team, providing exceptional clinical leadership and support. What You Bring to the Table: Essential Qualifications: · This position reports to the Administrator and Social Services Director. Conduct psychosocial assessments of patients. · Develop individualized care plans based on patient needs.· · Coordinate with healthcare teams to ensure comprehensive patient care. · Advocate for patients' needs and rights within the healthcare system. · Assist patients in navigating the healthcare system and accessing resources. · Facilitate discharge planning and continuity of care for patients. · Educate patients and families about healthcare options and resources. · Collaborate with community organizations to connect patients with additional services. · Document patient interactions and maintain accurate records. · Participate in multidisciplinary team meetings to discuss patient care. · Assist patients with financial concerns and connect them to relevant resources.
