Clinical Supervisor

Thrive Skilled Pediatric Care's purpose is simple, to provide high quality clinical home care to medically fragile children so they can grow and flourish to their full potential. The Clinical Supervisor is responsible for managing the provision of all therapeutic care and services provided to patients and oversees all aspects of coordination of care for these patients from admission through discharge of service. Is accountable for assessing, planning, implementing, coordinating, monitoring and evaluation of the care and services provided to our patients. The Clinical Supervisor is responsible for the clinical supervision of field clinical team members.

Hiring Requirements

A Bachelor's degree in health care administration or a related field. Holds a current nursing license in good standing as a Registered Nurse in the state of practice. Have a minimum of two (2) years of recent, verifiable, related supervisory clinical experience as an RN, including at least one (1) year preferably in a home care setting. Comprehensive understanding of issues related to the delivery and coordination of home healthcare and ability to problem solve issues effectively. Able to prioritize and handle multiple tasks of critical importance simultaneously while maintaining a professional and friendly demeanor. Must be able to multi-task while working in a fast-paced environment. Ability to work with people in such a manner as to build high morale, trust and group commitment to goals and objectives. Ability to build trusting relationships with referral sources, patients, caregivers, and all members of patient's care team. Must be well organized and demonstrate effective time management skills. Must be professional and able to effectively communicate both orally and in writing. Proficient in Microsoft Office; Excel, Word, Outlook, and web-based applications. Ability to read, write and effectively communicate in English.

Essential Responsibilities

Responsible for care coordination of patients from admission through discharge of service. Accountable for assessing, planning, implementing, coordinating, monitoring and evaluating to ensure the provision of safe, high quality care and services are provided to our patients.

Collaborate with Clinical Director/Location Director/Area Clinical Director to ensure that clinicians provide care and services in accordance with all clinical standards of practice, Thrive SPC policies, procedures and regulatory, accrediting agency guidelines. Act as a clinical lead for clinical care team and responsible for the clinical oversight and supervision of field RN, LVN/LPN. Act as a patient advocate including initiation of appropriate referrals for additional services as needed to ensure the highest quality outcome for the patient. Collaborate with outside providers including Case Managers, Discharge Planners, Physicians and DME to ensure a smooth, safe, and timely transition of patients from hospital to home. Perform initial assessments and ongoing reassessments to identify all patient needs and develop a comprehensive, individualized plan of care, with written actions, objectives, and goals including timeframes. Ensure timely submission of all required paperwork and documentation for authorization and reauthorization according to MCO and Private Insurance guidelines. Ensure all physician orders and Plans of Care are obtained timely according to policy and regulations. Ongoing Coordination of Care with all members of patient care team. Perform initial assessments for admissions, 60-day Supervisory Visits, Resumptions of Care, Recertifications and Discharges and make appropriate changes to patient's Plan of Care per physician orders and agreement of family. Perform daily QA and approval of patient nursing notes, incident reports, infection control reports, complaints, and physician orders. Escalate as appropriate to Area Clinical Director/Clinical Director and/or Location Director. Evaluate each patient's plan of care at least every 60 days to determine effectiveness and appropriateness of current plan, modify plan as appropriate in agreement with patient, patient's family, and physician. Actively participate in the hiring process for clinical team members. Responsible for the orientation, education, training, coaching, mentoring, and in-servicing of field and office staff to meet patient care and service needs. Participate in afterhours clinical on call rotation

Knowledge and Skills

Knowledge and understanding of the Nurse Practice Act for the state in which you are practicing. Knowledge and understanding of all Thrive SPC policies and procedures, ACHC Standards and State regulations. Ability to perform and instruct on all clinical skills outlined in Thrive SPC's competency tool. Strong clinical judgement and critical thinking skills. Effective and respectful communication skills both verbal and written. Ability to maintain a customer focus in every interaction.

Attention to detail and results oriented. Ability to work effectively independently and in a team environment. Strong interpersonal skills

Travel Requirements

  • Travel to client homes and office locations

  • May be exposed to dust, gas, fumes, and odors when traveling.
  • The physical demands of this job require constant visual and auditory requirements and sitting, keying and mouse input, occasional standing, walking, simple grasping, bending, squatting, kneeling, writing, pushing, pulling, reaching above and below the shoulder level.
  • Position requires occasional lifting demands of up to 5 pounds and occasional carrying demands of up to 10 pounds up to 50 feet.
