Travel Home Health Physical Therapist Case Manager jobs in Oak Brook, IL let you work remotely after a brief onsite orientation. You’ll manage adult and geriatric general med/surg patients, seeing about 6 visits per day within your coverage area. Shift 8hr Day Shift (40) Shift Information 8am start On Call, Call Back Requirements none Weekend Rotation Every 3rd weekend Holiday Requirements none Floating Requirements within Central Metro E coverage area Supplies and equipment are delivered to your home, and all meetings are virtual. Required qualifications include 2 years of PT home health experience with adult/geriatric patients, active Illinois PT license, BLS (AHA), valid driver’s license, auto insurance, and OASIS experience.
