Key Responsibilities

• Maintain adequate drug supply, oversee procurement of medications, manage controlled substance documentation, and ensure compliance with all applicable Federal and State Pharmacy laws, rules, and regulations.
• Prepare, sterilize, label, and dispense parenteral and non-parenteral medications; verify sterile product labeling and expiration dates; fill and check unit dose charts.
• Monitor patient drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose; detect and report suspected adverse drug reactions.
• Conduct monthly inspections of patient care units and pharmacy areas; ensure cleanliness and order of the pharmacy; maintain refrigerator temperature logs.
• Participate in medication use review, Pharmacy and Therapeutics Committee meetings, antimicrobial stewardship, pharmacokinetic dosing management, and anticoagulation dosing management and monitoring.
• Provide discharge counseling to patients and families; gather prior-to-admission home medication lists; complete medication reconciliation for discharge medication lists.

Required Qualifications

• Active and unrestricted Pharmacist license in the state of Ohio.
• Working knowledge of all areas of pharmaceutical services including drug procurement, dispensing, documentation, and regulatory compliance.
• Proficiency with EPIC electronic health records system.
• Strong clinical knowledge including antimicrobial stewardship, interpretation of culture results, and management of common infectious disease states (UTI, pneumonia, SSTI, etc.).
• Experience with pharmacokinetic dosing management and anticoagulation dosing management and monitoring.

Preferred Experience / Skills

• Experience with medication reconciliation processes including gathering home medication lists from patients, families, or outpatient pharmacies.
• Familiarity with JCAHO accreditation standards and quality improvement initiatives.
• Experience providing patient and family education regarding discharge medications.
