A well-established skilled nursing and rehabilitation center in the Wayne, NJ area is seeking a compassionate, dedicated, and experienced Social Worker to join our interdisciplinary team. This is an excellent opportunity for a professional who is passionate about resident advocacy, discharge planning, family support, and enhancing quality of life for residents in a post-acute and long-term care setting.

Provide psychosocial support to residents and their families
Conduct social service assessments and maintain accurate documentation
Assist with care planning and participate in interdisciplinary team meetings
Coordinate discharge planning and transitions of care
Facilitate resident and family meetings as needed
Assist residents in accessing community resources and support services
Address concerns related to resident rights, adjustment, and quality of life
Support admissions and orientation processes when necessary
Ensure compliance with federal, state, and facility regulations
Bachelor's Degree in Social Work required
Social work experience in a skilled nursing facility, rehabilitation center, healthcare setting, or long-term care environment preferred
Knowledge of discharge planning, care coordination, and resident rights
Strong communication, organizational, and problem-solving skills
Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members
Experience with electronic medical records preferred
