16-Hour Night Shifts

We are currently seeking an experienced LPN/LVN – Long Term Care (LTC) for a respected long-term care and skilled nursing facility near Claremont, New Hampshire. This opportunity is ideal for nurses with strong LTC experience who are passionate about providing compassionate, resident-centered care in a collaborative environment.

Position Details

Position: Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN)
Location: Claremont, NH
Setting: Long Term Care / Skilled Nursing Facility
Shift: Nights
Schedule: 48 Hours Per Week
Shift Length: 16 Hours
Shift Hours: 3:00 PM – 7:00 AM
Start Date: June 30, 2026
Assignment Length: 13 Weeks
Weekend Requirement: Every Other Weekend
On-Call Requirement: None
Floating: Must Float to Units Where Experienced

Responsibilities

  • Provide comprehensive nursing care to long-term care and skilled nursing residents.
  • Observe residents and promptly report changes in physical or mental condition.
  • Administer medications and treatments as prescribed and document accurately in electronic medical records.
  • Receive, transcribe, and implement physician orders.
  • Perform wound care, including management of wound vacs when applicable.
  • Administer oxygen therapy and enteral feedings in accordance with facility policies.
  • Supervise and support nursing assistants while ensuring quality resident care.
  • Assist with admissions, transfers, and discharges.
  • Communicate effectively with physicians, residents, families, and interdisciplinary teams.
  • Monitor infection control practices and maintain a safe resident environment.
  • Complete shift reports, resident documentation, and required nursing records.
  • Participate in staff education, orientation, and quality improvement initiatives.

Requirements

  • Active New Hampshire LPN License (Must Be Active and In Hand at Submission)
  • Minimum 3 Years of Current LPN Experience (Within the Last 6 Months)
  • Current BLS Certification (AHA)
  • IV Certification Required (Must Obtain During Onboarding if Not Currently Certified)
  • Must Be Willing to Float to Units Where Experienced
  • COVID-19 Vaccination Required (No Exemptions Accepted)
  • Previous Travel Experience Highly Preferred
  • Rural or Small Facility Experience Highly Preferred
  • Strong Long-Term Care and Skilled Nursing Experience Required
