Job description

IHCare+ is a mobile integrated healthcare company that partners with health systems, payors, and community agencies to keep high‑risk patients safe at home. Our paramedics deliver patient‑centered, in‑home care that blends EMS expertise with care management, chronic disease support, and social needs navigation.

Who is IHCare+
IHCare+ focuses on Mobile Integrated Health (MIH) and community paramedicine services in and around Ohio, supporting patients after hospital or ED visits and those with complex, chronic conditions. We work as an extension of patients care teams closing gaps between clinic visits through proactive home visits, telehealth-supported care, and targeted interventions that reduce unnecessary hospital visits and avoidable readmissions.
Our teams operate in a highly collaborative environment, working alongside nurses, advanced practice providers, care coordinators, and social service partners to deliver the right care, in the right setting, at the right time, with the right provider.

What Will You do as a Community Paramedic with IHCare+
IHCare+ paramedics provide scheduled, non‑emergent home visits focused on assessment, early intervention, and patient coaching rather than traditional transport‑driven EMS response. Typical activities include head‑to‑toe assessments, medication review, vitals and diagnostic checks, chronic disease monitoring, and escalation or teleconsultation with partnering clinicians when needed.
Our Community Paramedics also help identify and address barriers like transportation, health literacy, and access to primary care or behavioral health services, connecting patients with community resources and support programs. Documentation is completed in an electronic health record, supporting data‑driven quality improvement and program outcomes tracking.

Why Paramedics Choose IHCare+

  • Work at the top of your certification in an advanced, community‑based paramedicine model.
  • Build ongoing relationships with a defined patient panel, seeing the impact of your care over time in reduced hospital use and improved quality of life.
  • Practice in a structured, protocol‑driven program with strong medical oversight, clear clinical pathways, ongoing education in MIH and community paramedicine.
  • Join a culture that values clinical quality, safety, and continuous improvement, with paramedic input shaping workflows, protocols, and new service lines.

The Community Paramedic is responsible for providing in-home and mobile healthcare services to underserved and under-resourced populations. This role includes conducting health assessments, delivering care, offering chronic disease management, assisting with care coordination, and addressing social determinants of health. The Community Paramedic will collaborate with healthcare providers, Managed Care Organizations (MCOs), and social service agencies to support improved health outcomes and reduce hospital readmissions. Care responsibilities also include advanced procedures such as medication administration, IV placement, blood draws, wound care, and setting up telemedicine technology for remote consultations.

  • Manage and support patients with chronic conditions such as diabetes, asthma, COPD, hypertension, CHF, and behavioral health diagnoses.
  • Conduct point-of-care lab testing, blood cultures, wound evaluations, and geriatric assessments.
  • Collaborate with community resources, caregivers, and healthcare professionals to coordinate care and navigation services.
  • Maintain compliance with HIPAA, infection control protocols, and universal precautions.

Medical Equipment and Technology:

  • Deploy and troubleshoot in-home monitoring equipment and tech packs for virtual physician interactions.

Documentation and Reporting:

  • Document all patient interactions, assessments, care plans, and outcomes in electronic health records.
