The ideal candidate enjoys building relationships, solving problems, and working both independently and as part of a team.QualificationsAssociate degree in Human Services or a related field, or equivalent combination of education and experienceExperience in human services, case management, advocacy, education, healthcare, or a related field preferredStrong communication, organization, and problem-solving skillsAbility to work independently while supporting individuals in community settingsPassion for inclusion, advocacy, and empowering othersPreferred QualificationsBachelor's degree in a related fieldExperience working with individuals with disabilities or underserved populationsFamiliarity with community resources and support servicesLived experience with a disability Additional InformationTravel throughout New Hampshire is requiredThis role includes a combination of office, remote, and community-based work Not sure if you meet every qualification? We carry out our mission through our Home Care Services, Community-Based Disability Supports and Employment Services. With the help of our local New Hampshire communities, we assist people with the tools for living life independently, so they can change their lives for the better.
