Case Assistance Worker Ravenswood Older Adult Center – HANAC, Inc.

Location:

  • Astoria, NY Schedule: 28 hours per week Monday–Thursday: 9:00 AM – 3:00 PM Friday: 8:00 AM – 3:30 PM Pay: $21.22 per hour (based on experience) About the Role HANAC is seeking a compassionate and organized Case Assistance Worker to support older adults by connecting them with benefits, services, and community resources.
  • This position works closely with center participants to assess needs, provide referrals, maintain records, and assist with accessing programs that promote independence and well-being.
  • Key Responsibilities Conduct participant assessments and identify service needs.
  • Provide referrals, follow-up services, and resource coordination.
  • Assist with benefit, housing, and service applications.
  • Maintain accurate case records and data entry in required systems.
  • Conduct wellness calls and participant outreach.
  • Collaborate with community agencies and service providers.
  • Participate in staff meetings, training, and other program activities.
  • Qualifications Associate's Degree required; Bachelor's Degree preferred.
  • Experience in social services, case management, or senior services preferred.
  • Bilingual English/Spanish or English/Chinese preferred.
  • Strong communication, organizational, and computer skills.
  • Experience with Microsoft Office and data entry systems.
