Job Title: Licensed Practical Nurse (LPN)
Reports to: Unit Manager
Overview:
Van Duyn Center for Rehabilitation and Nursing is a 513-bed skilled Nursing facility atop Onondaga Hill in Syracuse, NY. The Van Duyn team is committed to providing quality care and diversified services with a professional team of caregivers.
Duties/Responsibilities:
· Medication administration
· Charge nurse role for oversight of CNA’s to monitor care provided
· Monitor effectiveness of the patient/resident plan of care
· Participate in the care planning process
· Reporting to nurse manager/nursing supervisor/ADON
Requirements/Qualifications:
· Valid LPN Practical Nurse License
· CPR Certification
· New Graduates are welcome to apply No Experience necessary
