Brentwood Behavioral Healthcare is a -bed acute psychiatric care facility located in Flowood, Mississippi. We are accredited by the Joint Commission of Accreditation of Healthcare Organizations and certified by Medicare, Medicaid and TRICARE®. We provide psychiatric treatment for depression, bipolar disorder, anxiety, schizophrenia, phobia and more. It is important these issues get treated so individuals are able to lead a healthy and happy life.

Position Summary:The Care Navigator serves as a vital liaison between inpatient treatment, outpatient services, community providers, patients, and families to ensure smooth transitions of care. This position focuses on patient engagement, discharge coordination, continuity of care, community outreach, and outpatient program growth. The Care Navigator assists patients diagnosed with Serious Emotional Disturbance (SED), Alcohol & Drug (A&D) disorders, and Adult Mental Health conditions while promoting ongoing treatment participation and reducing readmissions.

Minimum: High school graduate or equivalent with one to three years of related work experience.

Preferred: Associates’s Degree in Mental Health related field and three to five years of experience in related work experience.
