Are you organized, people-focused, and passionate about helping patients get the care they need? Join our team as an Hospice Clinical Sales Liaison and play a key role in guiding patients from hospital to home. You'll be the first point of contact for patients, families, and referral partners — coordinating care, verifying coverage, and setting the foundation for an excellent home health experience. The ideal candidate is clinically knowledgeable, ethical, and driven professional who excels at building referral relationships, educating healthcare partners, and supporting compassionate, patient-centered hospice care while contributing to organizational growth.
