Job description

At Best Home Health Providers Inc., we've gone from treating 10 patients to thousands of patients all throughout the Bay Area and beyond in just 6 years. We've won awards for our high-quality care with patients and continue to grow rapidly. At Best, you and the patients aren't a number.

Our mission is to ensure a smooth transition for patients returning home from medical facilities by offering expert guidance throughout their recovery journey.

Are you organized, people-focused, and passionate about helping patients get the care they need? Join our team as an Hospice Clinical Sales Liaison and play a key role in guiding patients from hospital to home. You'll be the first point of contact for patients, families, and referral partners — coordinating care, verifying coverage, and setting the foundation for an excellent home health experience. The ideal candidate is clinically knowledgeable, ethical, and driven professional who excels at building referral relationships, educating healthcare partners, and supporting compassionate, patient-centered hospice care while contributing to organizational growth.

We value teamwork, empathy, and a commitment to high-quality service, all aimed at putting patients and their families at ease during their recovery. If you are passionate about making a positive impact in patients' lives and working in a supportive and family-oriented atmosphere, we encourage you to join us in providing exceptional home health care.

Please note that this is a exempt role with an annual salary of $85k plus commission.

Schedule:

  • Monday through Friday, with occasional evenings or weekends for events, urgent referrals, or community outreach.

Key Responsibilities:

  • Build and maintain referral relationships with hospitals, skilled nursing facilities, assisted living communities, physicians, case managers, discharge planners, and other community partners.
  • Educate healthcare professionals, patients, and families about hospice services, eligibility, benefits, and the admission process.
  • Identify appropriate hospice candidates and coordinate with the admissions team to facilitate smooth transitions of care.
  • Provide clinical guidance on hospice appropriateness, disease progression, symptom management, and goals-of-care discussions.
  • Conduct sales calls, presentations, educational in-services, networking events, and community outreach.
  • Collaborate with clinical and operational teams to support patient growth and quality care.
  • Monitor referral activity, market trends, and growth opportunities using CRM tools.
  • Ensure compliance with HIPAA, Medicare regulations, and ethical referral practices.
  • Achieve referral, relationship development, and territory growth goals.
  • Consist meeting of monthly sales quotas.

Requirements

  • Clinical background preferred (RN, LVN/LPN, Social Worker, or other healthcare experience).
  • Experience in hospice, home health, post-acute care, senior care, or healthcare sales is preferred.
  • Bachelor's Degree in Business, Marketing, Healthcare, or related field.
  • Strong knowledge of hospice services and referral processes.
  • Excellent communication, presentation, and relationship-building skills.
  • Compassionate, organized, self-motivated, and comfortable discussing end-of-life care.
  • Valid driver's license, reliable transportation, and local travel required.
  • Proficiency with CRM systems and Microsoft Office.

Preferred Experience:

  • Established relationships with local hospitals, SNFs, ALFs, physicians, and case managers.
  • Proven success growing referrals, admissions, or territory.
  • Knowledge of Medicare hospice benefits and hospice levels of care.
  • Experience delivering clinical presentations and educational programs.

Benefits

  • Competitive salary with performance-based bonus
  • Health, dental, and vision insurance
  • Generous paid time off
  • 401(k) with a match
  • Mileage reimbursement
  • Phone/laptop or technology reimbursement
  • Ongoing training and professional development
  • Supportive leadership and career growth opportunities
