Reports To Director of Pharmacy

Position Summary

The Clinical Coordinator – Medication Safety Pharmacist is responsible for coordinating and advancing the organization's medication safety program. This position collaborates with interdisciplinary teams to identify medication-use risks, implement safety initiatives, improve medication-use processes, support regulatory compliance, and promote a culture of medication safety throughout the organization.

Primary Responsibilities

• Coordinate and support the organization's medication safety program and related quality improvement initiatives.

• Review, investigate, and analyze medication-related events, adverse drug events, near misses, and other medication safety concerns.

• Develop, implement, and evaluate medication safety strategies to reduce risk and improve patient outcomes.

• Monitor medication safety metrics, analyze trends, and communicate findings to leadership and committees.

• Support compliance with regulatory, accreditation, and medication management standards.

• Coordinate the review, development, implementation, and maintenance of medicationrelated policies, procedures, and standards.

• Facilitate and participate in medication safety, Pharmacy and Therapeutics (P&T), quality, and other interdisciplinary committees as assigned.

• Coordinate implementation of medication safety recommendations, alerts, recalls, and best practices from internal and external sources.

• Assess medication safety implications of new medications, technologies, workflows, formulary changes, shortages, and recall events.

• Develop and provide medication safety education, training, and communications to healthcare professionals.

• Collaborate with pharmacy, nursing, medical staff, quality, risk management, and informatics teams to improve medication-use processes.

• Serve as a resource and subject matter expert for medication safety-related issues.

• Participate in the education and development of pharmacy residents and students through precepting, mentorship, and project support.

• Participate in emergency preparedness, medication shortage management, and business continuity planning activities related to medication-use systems. • Perform other duties and responsibilities as assigned.

Qualifications Required:

• Doctor of Pharmacy (PharmD) degree from an ACPE-accredited school of pharmacy.

• Current Connecticut Pharmacist License or eligibility for licensure. • Completion of an ASHP-accredited PGY1 Pharmacy Residency.

• Minimum of three (3) years of hospital pharmacy experience.

• Previous participation in multidisciplinary committees, workgroups, or quality improvement initiatives.

Preferred • Board Certification (e.g., BCPS).

• Experience with medication safety, quality improvement, accreditation, or regulatory compliance activities.

Additional Requirement

• Completion of a recognized Medication Safety Certificate Program within six (6) months of appointment to the position. Knowledge, Skills, and Abilities

• Knowledge of medication safety principles, medication-use systems, and patient safety practices.

• Knowledge of applicable regulatory, accreditation, and medication management standards.

• Ability to analyze data, identify trends, and develop improvement strategies.

• Ability to investigate medication-related events and recommend system-based solutions.

• Strong verbal, written, presentation, and interpersonal communication skills.

• Ability to facilitate interdisciplinary committees and projects.

• Ability to manage multiple priorities in a dynamic healthcare environment.

Supervisory Responsibilities

This position does not have direct supervisory responsibility but provides coordination, guidance, and subject matter expertise for medication safety initiatives and interdisciplinary workgroups
