About the Position

Description

The Care Coordinator is responsible for managing patient care during episodes of illness or treatment, collaborating with patients, families, and healthcare teams. They assess, plan, intervene, and evaluate care while advocating for patients, ensuring effective resource utilization, and maintaining communication throughout the care process.
