Program Supervisor

The program supervisor will play an active role in the program design, development, implementation, and support of PHM initiatives. The Program supervisor will oversee day to day operations over population health Coordinators, manage projects and other assignments in the PHM department. Analyze data to identify and address care gaps within the patient population. Collaborate with Practice Administrator and Leading Provider to build strong and collaborative relationships with physicians, care team members and payers. Oversee clinical population health strategies, annual goals and objectives in the value-based healthcare environment.

Hours: 8:30am-5:30pm
Qualifications: Education and/or Experience: High school diploma and LVN license required Experience in data collection and interpretation of data, know computer technology and have an understanding of the healthcare industry. One year experience implementation and evaluation of quality measures
Certificates, Licenses, and Registrations: Current California Licensed Vocational Nurse (LVN) license required.
Essential Functions: Provides comprehensive analysis, barriers and opportunities and presents results of improvement and ongoing performance measures. Assist with data collection, analysis and reporting Assist with PHM regulatory requirements. Collaborates with clinic operations to improve the operations of strategic planning, patient satisfaction, quality assurance and other operational improvements Supports development of quality initiatives and other programs to continually improve quality, decrease cost, provide outstanding patient experience incorporates initiatives that will increase patient engagement Serves as a designated clinical leader for Population Health program Monitoring and measuring productivity. Providing training to staff, resolving staff issues and identifying areas for improvement. Completes other projects and duties as assigned
