We are seeking a dedicated and compassionate Social Worker to join our team at Pawtucket Falls Health Care in Pawtucket, Rhode Island. As a Social Worker at Pawtucket Falls, you will play a vital role in providing exceptional care and support to our residents, while also making a positive impact on their lives.
About the Role:
As a Social Worker, you will be responsible for assessing the social and emotional needs of our residents, providing counseling and support to help them cope with challenging situations, and collaborating with other healthcare professionals to develop and implement individualized care plans. Your expertise will enable you to identify and address potential social and emotional issues, ensuring that our residents receive the comprehensive care they need to thrive.
Responsibilities:
Assess the social and emotional needs of residents, including their medical, psychological, and emotional well-being
Develop and implement individualized care plans to address the unique needs of each resident
Provide counseling and support to residents, families, and caregivers to help them cope with challenging situations
Collaborate with other healthcare professionals, including physicians, nurses, and therapists, to develop and implement comprehensive care plans
Identify and address potential social and emotional issues, such as depression, anxiety, and isolation
Maintain accurate and up-to-date records of resident care and services

Requirements:
A bachelor's degree in Social Work or a related field
