Nurse Practitioner - Peoria, Illinois

As a Nurse Practitioner (NP) or Physician Assistant (PA), you'll play a crucial role in building strong relationships with our facilities, maintaining a patient-centered approach, and prioritizing proactive measures for enhanced physical health and well-being.
About the Role:
As part of our comprehensive preventative care program, you'll deliver high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. We're seeking an experienced NP or PA with a background in Geriatrics or Internal Medicine to join our team.
Responsibilities:
• Deliver high-quality preventative care to residents in skilled nursing facilities
• Combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation
• Build strong relationships with the facility and be a collaborative team player
• Maintain a patient-centered approach and prioritize proactive measures for enhanced physical health and well-being
Requirements:
• Active NP or PA license (in the state of practice)
• 1+ year of experience in Geriatric care and Documentation review
• Excellent communication, collaboration, and teamwork skills
• This is a part-time position of 19.5 hours per week
