Job Title Physical Therapist Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our team in a skilled nursing facility located in Lawrence, Massachusetts. In this role, you will be instrumental in enhancing the functional abilities and independence of our residents, partnering closely with them and their caregivers to achieve optimal outcomes. The Physical Therapist will be responsible for comprehensive evaluations, developing individualized treatment plans, and implementing therapeutic interventions. This position requires a collaborative approach, working within an interdisciplinary healthcare team to ensure seamless, resident-centered care and adherence to all regulatory standards. Key Responsibilities Perform comprehensive evaluations and therapeutic procedures under physician orders, observing all precautions and contraindications. Establish individualized therapeutic plans of care, including functional long- and short-term goals, frequency, duration, and appropriate therapeutic modalities. Collaborate with residents, primary caregivers, referring physicians, and the interdisciplinary healthcare team to integrate input into the plan of care and facilitate discharge planning. Monitor and evaluate treatment effects, progressing or modifying treatment programs as necessary to achieve functional goals. Document all evaluative and therapeutic interactions accurately and timely in the Electronic Medical Record (EMR), ensuring clarity for the care team and compliance with reimbursement regulations (e.g., Medicare, Medicaid). Recommend and obtain appropriate assistive devices, providing thorough instruction on their use to residents and/or caregivers. Provide education to residents and caregivers, documenting teaching outcomes. Refer residents to Physical Therapist Assistants (PTAs) as appropriate and collaborate effectively with other therapy professionals. Supervise assistant-level staff, ensuring adherence to state mandates and professional standards, and address performance issues with the Director. Provide mentorship to new staff during orientation, ensuring completion of departmental checklists and basic competencies. Contribute to the development of new rehabilitation programs and services. Maintain a clean and organized clinical environment, assisting with routine equipment and space cleaning, and adhering to infection control procedures. Required Qualifications Bachelor's degree in Physical Therapy. Previous experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting. Preferred Qualifications Master's or Doctorate degree in Physical Therapy. Basic Life Support (BLS) certification. Skills & Competencies Proficiency in performing evaluative and therapeutic procedures and developing comprehensive treatment plans. Competence in recommending and instructing on the use of assistive devices. Strong understanding and compliance with Medicare, Medicaid, Private Pay, and other reimbursement regulations. Excellent verbal, written, and presentation communication skills. Strong interpersonal and customer service skills, with a compassionate and resident-centered approach. Ability to work collaboratively as a responsible and supportive member of an interdisciplinary team. Analytical skills with the ability to make sound clinical decisions. Proficiency in Electronic Medical Record (EMR) documentation and Microsoft Office Suite. Excellent time management and organizational skills. Ability to speak, read, write, and understand English fluently.
