Job description
Job Title Clinical Social Worker Job Summary This role is for a dedicated Clinical Social Worker specializing in Behavioral Health, located in Grand Rapids, Michigan. The successful candidate will provide essential social work services within a dynamic healthcare environment, focusing on patient well-being and support. The position involves working with diverse patient populations requiring behavioral health interventions, assessments, and care coordination. This is an excellent opportunity for a professional committed to making a significant impact on patient lives in a supportive setting. Key Responsibilities Conduct comprehensive psychosocial assessments for patients in a behavioral health setting. Develop and implement individualized care plans, including therapeutic interventions and discharge planning. Provide crisis intervention and support to patients and their families. Collaborate with interdisciplinary teams to ensure integrated patient care. Facilitate access to community resources and support services. Maintain accurate and timely documentation of patient interactions and progress. Required Qualifications Master's degree in Social Work (MSW) from an accredited program. Minimum of 1-2 years of recent experience as a Clinical Social Worker, preferably in a behavioral health specialty. Active Social Worker license or accreditation in the state of Michigan.
