Skilled Healthcare Facility -
Are you passionate about advocating for residents and supporting families through every stage of their healthcare journey? Our skilled nursing facility in the Cincinnati area is seeking a compassionate and organized Social Services Director to join our leadership team.
This is an opportunity to make a meaningful impact while working alongside an experienced interdisciplinary team dedicated to providing exceptional resident care.
Coordinate and oversee the facility's Social Services department.
Complete resident assessments, care plans, and required documentation.
Assist with admissions, discharges, and care transitions.
Provide support, counseling, and resources to residents and their families.
Participate in care conferences and interdisciplinary team meetings.
Ensure compliance with state and federal regulations.
Advocate for resident rights and quality of life.
Build positive relationships with residents, families, hospitals, and community partners.
Long-term care or skilled nursing experience required
Knowledge of state and federal regulations related to long-term care.
Excellent communication, organization, and documentation skills.
Compassionate, resident-focused approach to care.
Ability to work independently while collaborating with an interdisciplinary team.
