Inpatient Pharmacist Job Description Job Title Inpatient Pharmacist Job Summary We are seeking a dedicated and clinically-focused Inpatient Pharmacist for a 13-week contract assignment at a prominent medical center in Chico, California. This role is integral to providing comprehensive pharmaceutical care within a dynamic hospital setting, ensuring optimal patient outcomes through expert clinical judgment and efficient medication management. The successful candidate will operate within a decentralized pharmacy model, applying professional knowledge and distribution skills to identify, resolve, and prevent drug-related problems. This position requires adherence to established policies, procedures, and federal/state regulations, contributing significantly to the overall mission of the Pharmacy Department and the hospital. Key Responsibilities Provide comprehensive pharmaceutical care, including accurate medication dispensing and distribution. Review and verify medication orders for appropriateness, dosage, drug interactions, and contraindications. Monitor patient drug therapy for efficacy, adverse effects, and compliance, making recommendations as needed. Identify, resolve, and prevent drug-related problems through proactive intervention and collaboration with the healthcare team. Provide accurate and timely drug information to physicians, nurses, and other healthcare professionals. Participate in interdisciplinary rounds and patient care conferences as required in a decentralized setting. Ensure strict compliance with all federal, state, and local pharmacy laws and regulations, as well as hospital policies and procedures. Maintain accurate and complete patient medication records. Required Qualifications Minimum of one (1) year of hospital experience in an inpatient, decentralized pharmacy setting. Demonstrated proficiency with pharmacy computer programs and Windows operating systems. Proven ability to apply extensive drug knowledge and clinical judgment in a fast-paced environment. Excellent problem-solving abilities for identifying and resolving drug-related issues. Proficiency in medication order entry and verification systems. Exceptional communication and interpersonal skills for effective collaboration with healthcare teams. High level of attention to detail and accuracy in medication preparation and dispensing. Ability to work autonomously and as part of a team in a decentralized pharmacy model.
