Site Director for Brockton Program Location

Harbor provides medical, behavioral health, dental, and support services to more than 34,000 patients in Boston, the South Shore, and Cape Cod.

Harbor Health operates two Elder Service Plan Programs of All-Inclusive Care for the Elderly, a program that allows frail elders to live in their homes and communities. Our Elder Service Plan has two medical and day centers in Mattapan and Brockton, MA and cares for more than 580 participants. We are currently seeking a Site Director for our Brockton program location.

Role:

The Elder Service Plan (ESP) Site Director, under the general direction and supervision of the Senior Director of Utilization Management, oversees and manages the day-to-day clinical operations of the Elder Service Plan/PACE center in Brockton, MA. Working in conjunction with ESP's Department Directors and Managers, the Site Director serves as the PACE Program Manager at the center level, with primary accountability for the effectiveness, coordination, and regulatory compliance of the interdisciplinary team. The Site Director is responsible for ensuring the program's needs and goals are met by ensuring the smooth and seamless daily operations at the assigned site. Areas of responsibility include the day center, medical clinics, nurse triage, care coordination, nutrition, social work, recreation, and site operations.

Responsibilities:

  • Oversees hiring, training, development, and evaluation of new and established staff.
  • Develops processes and resources for escalation support in difficult clinical, behavioral, or operational situations; provides direct participant or family intervention as needed and supports team in additional training as warranted.
  • Takes full responsibility for comprehensive and consistent phone triage processes by triage nurses; ensures phones are staffed, reviews response and call times, audits notes for quality, and spearheads quality improvement initiatives.
  • Oversees recreation and activities at all sites; ensures activity programs enrich and enhance the lives of participants and Encourages creativity and independence within the activities departments to collaborate, develop new programming, and enrich participants' lives.
  • Interdisciplinary team and care management oversight and ensures participant rights, preferences, and goals are central to all care decisions.
  • Actively participates in and plays a leadership role in interdisciplinary team (IDT) meetings, guiding discussion to ensure decisions are documented, actionable, and reflect team consensus. Maintains focus on positive clinical outcomes while keeping sight of quality improvement and financial goals, with the overarching aim of optimizing quality of life for participants.
  • Develops, documents, trains, and implements Standard Operating Procedures (SOPs) and systems that are scalable and replicable and evaluates staff adherence and provides feedback as needed to promote improvement.
  • Develops and tests new technologies, clinical tools, and workflows to ensure understanding and support optimization; ensures direct reports have the resources needed to utilize new tools effectively.
  • Monitors staff adherence to compliance and regulatory processes to ensure they are completed and documented appropriately, follows up timely as needed when staff need support and correction and creates improvements to existing processes to improve consistency.
  • Develops, implements, and manages quality improvement efforts by setting departmental goals at regular intervals and tracking progress, conducting monthly report-outs about quality and utilization initiatives and demonstrates ability to track and understand key performance indicators.
  • Ensures staff ratios are aligned with participant/program demands.
  • Ensures continuity of operations with adequate staffing, supplies and other resources as needed.
  • Monitors clinical operations to ensure consistent compliance with established standards and local, state and federal regulations.
  • Ensures nursing on-call schedule is covered and supported.
  • Continuously evaluates the program's changing needs and makes necessary adjustments.
  • Actively participates in interdisciplinary team (IDT) meetings and serves as team lead as needed.
  • Supports and, as needed, directs the coordination of home health, rehabilitative, long-term care, and medical/specialty services according to participant care plans.
  • Assists in development of annual operational and capital budgets for departments of accountability and manages services to meet financial benchmarks.
  • In conjunction with the ESP Leadership, maintains compliance certification under CMS as well as any other state or federal laws or regulations.
  • Monitors operational compliance with Occupational Safety and Health Administration (OSHA) regulations and agency policies and procedures.
  • Maintains professional affiliations and any required certifications.

Requirements:

  • Bachelor's degree and current licensure in a healthcare or human services discipline required; eligible disciplines include nursing, rehabilitation therapy, social work, nutrition, or other health professions relevant to the interdisciplinary care model; RN licensure preferred
  • Current Massachusetts licensure in a clinical discipline required, including Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), Registered Dietitian (RD), or Licensed Clinical Social Worker (LCSW/MSW as applicable to licensure requirements), Registered Nurse (RN) is strongly preferred.
  • Current BLS (Basic Life Support for Healthcare Providers) Certification required
  • 3-5 years of progressively responsible experience in management and supervision of staff
  • 1-2 years of experience in healthcare, at least one year in a geriatric care setting
  • Knowledge of the physical, mental, and social needs of the elderly
  • Demonstrated effective leadership and decision-making skills
  • Proven aptitude for developing and implementing workflows and improvement initiatives
  • Ability to effectively and efficiently plan, prioritize, and follow-up on delegated responsibilities
  • Superior verbal and written communication skills
  • Strong group facilitation skills
  • Intermediate knowledge of Microsoft Office 365 including Word, Excel, Outlook and Teams and use of Electronic Medical Records (EMR), EPIC desired
  • Working knowledge of PACE (Program for All Inclusive Care for the Elderly) preferred
  • Working knowledge of state's healthcare system, local health care, and aging networks preferred
  • Working knowledge of Quality Improvement and care management systems preferred

When determining actual base salary and/or rate, several factors may be considered as applicable (e.g. location, years of relevant experience, education, training, and other factors as permissible by law). As such, most offers will not be at the top of the salary range.
