Mental Health Associate

Responsible for providing psychosocial rehabilitative services to individuals with severe and persistent mental illness in an adult day program.

Requirements:

• High school diploma or GED
• 1-3+ years of relevant experience
• Adult CPR and First Aid certifications
• Driver's license and clean driving record

Duties:

• Teach and assist members with various activities of daily living skills
• Maintain a minimum caseload of at least 15 clients, and complete documentation on time
• Work in a team environment
• Effectively communicate through written and verbal format
• Perform case management duties which include the procuring of, and linkage to community resources
• Ability to drive a 7-12 passenger vehicle
• Conduct one-to-one and group sessions related to teaching activities of daily living skills
• Conduct outreach to clients in the community when needed or indicated on the client's IRP
• Escort and transport members to various appointments/outings as needed
