Job description
FACT Clinical Coordinator
Open Door is seeking a FACT Clinical Coordinator to work in Yonkers, NY. Prior experience with FACT/ACT is preferred.
FACT Clinical Coordinator will be the director of the FACT team who provides supervision to a dynamic/multidisciplinary team that services justice-impacted individuals across Westchester County who are referred by County SPOA system.
Responsibilities
- Work collaboratively with the RJ Director to set the mission/vision of the program.
- Provide clinical and individual supervision to the team members.
- Work with the RJ Director to manage the FACT budget and work with finance to ensure compliance with spending as outlined in grant.
- Oversee billing, work with finance to ensure we are optimizing all opportunities and ensure fiscal stability.
- Ensure that team members complete the required trainings and incorporate ongoing training opportunities to foster professional and personal development within the team.
- Work collaboratively with the RJ Director to hire new team members within FACT.
- Coordinate and facilitate/participate in regularly scheduled meetings with OMH, CPI and/or DCMH.
- Review case notes for appropriate content and sign off on case notes as necessary.
- Oversee progression of cases for follow-through while open and monitor cases for appropriate and timely closure.
- Provide clinical support to the team, especially lead clinicians, regarding appropriate interventions/case management.
- Ensure that the medical team within FACT has appropriate support.
- On call 24hrs, in rotation with other team members, to provide clinical support.
- Work with the RJ Director and team to build relationships with PDs/EMS/community stakeholders within areas where clients are heavily populated.
- Assess, engage, de-escalate and stabilize people and situations.
Requirements
- NYS registered licensed LMSW, LMHC (Required), LCSW (Preferred), CASAC a plus
- 5-10 years in related field
- Valid NYS driving license is required.
- Fluent in English and Spanish (Preferred)
- Excellent clinical experience with mentally ill, substance abuse afflicted population
- Desire to work in underserved communities
- Outstanding clinical, assessment, and diagnostic skills
- Proficient in the use of computers, and specifically with electronic medical records
- Excellent written and verbal communication skills with a culturally diverse population
- Team-focused collaborator
- Passionate about providing high-quality care to low-income, underserved residents in a mental health setting
- Excellent interpersonal skills both verbally and in writing
- Highly organized, selfdirected
- Engaged in learning and continuous quality improvement
- Resilient
FACT team members are expected to provide direct care to participants in community where they reside: homes, shelter setting, streetside, etc.
