Job Summary

The Comprehensive Care Advanced Care Practitioner (ACP) will be an integral part of the clinical team, reporting to the Market Medical Director. This role focuses on improving outcomes for senior members by conducting comprehensive visits designed to identify and close outstanding care gaps, address chronic conditions, and ensure members receive coordinated, high-quality care.

The ACP will see priority members either in clinic or through an approved telehealth platform and will work collaboratively with physicians, care teams, and support staff to provide proactive, patient-centered care.

Essential Duties & Responsibilities

• Review patient charts in advance of visits to identify clinical, diagnostic, and quality opportunities and develop a plan to address care gaps.

• Collaborate with clinic leadership and care team members to prioritize patient scheduling based on outstanding healthcare needs.

• Conduct comprehensive patient visits in clinic or through approved telehealth platforms.

• Review and address historical and suspected medical conditions identified through clinical assessment tools.

• Address quality measures and preventative care opportunities, including appropriate screenings and point-of-care testing when available.

• Interpret evidence-based practice screenings and diagnostic results.

• Develop and maintain an accurate list of active medical conditions using clinical judgment and assessment findings.

• Document comprehensive assessments and treatment plans for chronic conditions within the electronic medical record (EMR).

• Manage routine medication refills according to established clinical guidelines and protocols.

• Coordinate appropriate follow-up care and communicate effectively with the patient’s care team to promote continuity of care.

• Recognize urgent and emergent medical situations and respond appropriately.

• Educate patients on disease management, preventative health screenings, medication adherence, and healthy lifestyle choices.

• Participate in training sessions, team meetings, and quality improvement initiatives.

• Demonstrate strong understanding of clinical documentation improvement (CDI) practices and support team education when needed.

• Complete and finalize documentation in a timely manner according to organizational standards.

• Deliver exceptional patient experiences and maintain strong patient satisfaction outcomes.

Required Education & Experience

• Active Nurse Practitioner (NP) or Physician Assistant (PA) license, or ability to obtain prior to start date.

• Current national certification through ANCC, AANP, or NCCPA in one of the following areas:

  • Family Practice

  • Geriatrics

  • Adult Health

  • Adult-Gerontology Primary Care

  • Adult-Gerontology Acute Care

• Minimum of 1 year of clinical experience at the highest level of education completed.

• Experience working with senior populations preferred.

• Background in gerontology, cardiology, internal medicine, endocrinology, or primary care preferred.

• Ability to meet physical requirements of the role, including lifting up to 30 pounds, navigating various environments, and performing physical assessments.

• Willingness to travel within the licensed state when needed.

Knowledge, Skills & Abilities

• Experience working with electronic medical records (EMR).

• Strong understanding of ICD-10 coding and clinical documentation.

• Knowledge of diagnostic criteria and evidence-based medical practices.

• Familiarity with Medicare Advantage, HCC coding models, and quality measures (STARs) preferred.

• Ability to develop, implement, and evaluate individualized patient care plans.

• Strong clinical judgment and ability to work independently.

• Excellent communication skills with patients, families, and healthcare teams.

• Ability to work effectively with diverse patient populations and healthcare professionals.
