Certified Occupational Therapy Assistant (COTA) – In-Home/Outpatient Rehabilitation

Location: Alameda, Hayward, San Leandro, and Santa Clara Available
Job Type: Contract/Travel
Schedule: 8:00-4:30
Pay: $44-48/hr, depending on experience

We are seeking a Certified Occupational Therapy Assistant (COTA) to provide outpatient rehabilitation services to primarily older adult patients in their homes and residential settings.

This is a unique opportunity to deliver occupational therapy in the environment where patients actually live and function. You’ll work directly with patients to help improve independence, mobility, safety, and their ability to perform everyday activities—all while working under the supervision of a licensed Occupational Therapist.

The position offers a flexible schedule and varied work environment, with services provided in private residences and independent living settings.

What You’ll Do

As a COTA, you will implement individualized occupational therapy treatment plans established by the supervising Occupational Therapist and help patients improve their independence and quality of life.

Your responsibilities may include:

  • Provide individualized occupational therapy treatments under the supervision of an Occupational Therapist
  • Implement treatment plans established by the primary Occupational Therapist
  • Help patients improve functional skills and independence with activities of daily living
  • Educate patients, family members, and caregivers on treatment techniques and strategies
  • Collaborate with Occupational Therapists and other members of the rehabilitation team
  • Participate in patient care conferences, staffing meetings, and other required clinical meetings
  • Maintain accurate and timely clinical and billing documentation
  • Follow established patient treatment schedules
  • Monitor therapy equipment and report maintenance or repair needs
  • Provide compassionate, patient-centered care that respects each patient’s individual needs and rights
  • Participate in quality improvement and other clinical initiatives as required

Qualifications

  • Current Certified Occupational Therapy Assistant licensure in the state of practice, or proof of eligibility for licensure
  • Valid driver’s license or reliable independent transportation to and from patient residences
  • Strong organizational and time-management skills
  • Strong verbal and written communication skills
  • Ability to work independently while collaborating closely with the supervising Occupational Therapist and rehabilitation team
  • Commitment to providing compassionate, high-quality patient care

Physical Requirements

Candidates must be able to perform the essential functions of the position with or without reasonable accommodation, including:

  • Frequent standing and walking
  • Occasional to frequent sitting
  • Reaching, handling, fine manipulation, seeing, hearing, and communicating with patients
  • Climbing stairs, balancing, stooping, kneeling, crouching, crawling, and twisting as needed
  • Lifting, carrying, pushing, and pulling up to 50 pounds
  • Traveling to patient residences and working in varying home environments

Requirements

2 years of experience as a Occupational Therapist Assistant

Travel or Contract experience preferred

Active CA COTA License
