For nearly 35 years, Fourth Street Clinic has provided essential health care services to individuals experiencing homelessness in the Salt Lake City area. Position Overview
The Medical Case Manager (MCM) is a key member of the Medically Vulnerable Population (MVP) team, providing patient-centered case management for unhoused and medically vulnerable individuals. The MCM conducts needs assessments, develops personalized care plans, coordinates medical and behavioral health services, and facilitates access to supportive resources. This role ensures continuity of care, addresses social determinants of health, and advocates for patients to achieve optimal health outcomes.
