RN Case Manager Aegis Healthcare is expanding our Home Health and Hospice Nursing Teams! Your Role as RN Case Manager:As a Home Health & Hospice Nurse Case Manager, you'll be instrumental in providing exceptional care to patients in their homes. Your role will include:Conducting comprehensive physical, social, and psychological assessments.Collaborating with patients, families, and the Interdisciplinary Group to create individualized care plans.Managing care delivery and maintaining meticulous patient records.Acting as a resource for families, making proactive referrals, and fostering a holistic care experience. Qualifications We're Looking For:Active Registered Nurse (RN) license in Arizona (or state of practice).Previous Home Health or Hospice experience (preferred).Effective verbal, written, and time management skills.Valid driver's license, reliable transportation, and proof of current auto insurance.Ability to perform patient transfers with a full range of movement.Current CPR certification and negative TB test (we can assist).Valid AZ Fingerprint Clearance Card (we can help obtain it).
