Physical Therapist - Outpatient Job Description Job Title Physical Therapist - Outpatient Job Summary We are seeking a highly skilled and dedicated Physical Therapist for a 13-week contract assignment in an outpatient rehabilitation setting located in Hillsboro, Oregon. This role offers an opportunity to provide comprehensive physical therapy services to a diverse patient population. The successful candidate will be responsible for assessing patient conditions, developing individualized treatment plans, and implementing therapeutic interventions to restore function, reduce pain, and prevent disability. Key Responsibilities Conduct comprehensive patient evaluations, including medical history review, physical examination, and functional assessments. Develop and implement individualized treatment plans based on patient needs, goals, and evidence-based practice. Administer therapeutic exercises, manual therapy techniques, modalities (e.g., ultrasound, electrical stimulation), and other physical therapy interventions. Educate patients and their families on their condition, treatment plan, home exercise programs, and injury prevention strategies. Monitor patient progress, modify treatment plans as necessary, and document all evaluations, treatments, and discharge summaries accurately and timely. Collaborate effectively with physicians, occupational therapists, and other healthcare professionals to ensure coordinated and holistic patient care. Maintain a safe and clean working environment, adhering to all facility policies and procedures, as well as professional and ethical standards. Required Qualifications Active Physical Therapist (PT) license or accreditation in the State of Oregon. Minimum of 1-2 years of recent, relevant experience as a Physical Therapist.
