Certified Occupational Therapy Assistant (COTA) - Home Health - Riverside

This is a field-based home health position primarily serving patients throughout the Riverside area. The COTA will travel to patients' homes within the assigned territory to provide skilled occupational therapy services.

We are seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our home health team. This field-based position will primarily serve patients throughout the Riverside area and offers the opportunity to provide one-on-one care where it can make the greatest impactin the patient's home.

Position Summary

As a Home Health COTA, you will work under the direction and supervision of a licensed Occupational Therapist to help patients regain, improve, and maintain the skills needed to safely and independently perform daily activities.

You will provide individualized treatment in the home setting, reinforce established plans of care, educate patients and caregivers, and collaborate with the interdisciplinary care team to support positive outcomes and help patients achieve their highest level of independence.

What You'll Do

  • Provide skilled occupational therapy treatments in accordance with the Occupational Therapist's established plan of care.
  • Help patients improve their ability to perform activities of daily living, including dressing, bathing, grooming, meal preparation, mobility, and other functional tasks.
  • Assist patients in developing, recovering, and maintaining the skills needed for greater independence and safety in the home.
  • Implement therapeutic activities and exercises designed to improve strength, coordination, endurance, balance, and functional performance.
  • Reinforce patient and caregiver education regarding safety, adaptive techniques, home exercise programs, and use of assistive or adaptive equipment.
  • Observe and report changes in the patient's functional status, response to treatment, and progress toward established goals.
  • Communicate regularly with the supervising Occupational Therapist and other members of the interdisciplinary care team regarding patient progress and needs.
  • Identify potential safety concerns or barriers within the home environment and communicate findings to the Occupational Therapist.
  • Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
  • Help patients and families adjust to changes in functional abilities while promoting independence, dignity, and quality of life.
  • Participate in care coordination and collaborate with nursing, therapy, physicians, and other members of the patient's healthcare team.

Qualifications

  • Graduate of an accredited Occupational Therapy Assistant program.
  • Current and unrestricted Occupational Therapy Assistant license in the State of California.
  • Current CPR certification.
  • Minimum of one year of clinical experience preferred.
  • Home health experience strongly preferred, but we welcome candidates with strong clinical backgrounds who are interested in transitioning into home health.
  • Strong communication, organizational, and time-management skills.
  • Ability to work independently while collaborating effectively with the supervising Occupational Therapist and interdisciplinary team.
  • Reliable transportation, valid driver's license, and current automobile insurance.
  • Ability and willingness to travel to patients' homes throughout the assigned territory, primarily within the Riverside area.
  • Ability to complete clinical documentation accurately and within required timeframes.
