We are looking for an experienced and bilingual Patient Care Coordinator to join our growing team!
Requirements:
✔️ Bilingual English/Spanish REQUIRED
✔️ Experience coordinating patients and HHAs
✔️ HHAeXchange experience preferred
Responsibilities:
- Coordinate patient care and HHA services
- Assist with HHA staffing and schedules
- Communicate with patients, families, and caregivers
- Follow up on patient needs and schedule changes
- Maintain accurate patient documentation
- Support daily home health operations
