*This is a flexible PRN position for 2nd shift (Mon-Fri, 3pm-11pm).

We proudly title our LPNs as Coordinators of Health & Wellness, reflecting the collaborative role they play in our community and the important work they do to support our residents and families.
The Coordinator of Health & Wellness will provide clinical monitoring and support for residents with Alzheimer's/Dementia.
Ensure that all restorative, support and preventative measures are met in accordance with applicable state laws and regulations.
Experience with Alzheimer's/Dementia or behavioral health conditions, and a willingness to work with the elderly population are essential for this role.
