Location: San Diego, CA Travel: Up to 75%, including travel to Los Angeles Position Type: Grant-Funded through June 30, 2027 (high possibility of renewal, has been renewed 5 years in a row) Position Overview Eastridge Workforce Solutions is seeking an experienced Project Manager Family Nurse Practitioner (PMFNP) to support the Dr. Gloria J. McNeal Nurse Managed Clinic (GJMNMC) in San Diego. This is a unique opportunity for a mission-driven Family Nurse Practitioner to combine direct patient care, clinical leadership, program management, and community outreach while expanding access to healthcare for underserved and vulnerable populations.

The PMFNP will provide clinical and operational leadership across community-based healthcare programs, oversee clinic operations and staff, support grant-funded initiatives, and provide direct care to patients across the lifespan. The role serves populations experiencing significant health disparities, including individuals experiencing homelessness and other at-risk communities, as well as children receiving services in elementary school settings.

Responsibilities

• Provide primary care services to underserved and vulnerable populations across the lifespan
• Deliver healthcare services to individuals experiencing homelessness and other at-risk populations
• Support community-based healthcare initiatives throughout San Diego and Los Angeles
• Provide care in clinic, mobile, community, and elementary school settings
• Collaborate with interdisciplinary healthcare teams to improve patient outcomes
• Monitor patient care plans and support continuity of care

Clinic Operations & Leadership

• Manage day-to-day operations across multiple clinic and community locations
• Oversee clinic workflows, scheduling, and patient-centered service delivery
• Supervise Nurse Practitioners and Community Health Administration staff
• Support billing, credentialing, compliance, and quality improvement initiatives
• Manage Electronic Health Record systems and related operational processes
• Ensure culturally responsive, compliant, and high-quality care delivery

Grant & Program Management

• Support grant-funded initiatives, deliverables, and compliance requirements
• Collaborate with internal and external stakeholders to achieve program goals
• Assist with federal reporting, outcome tracking, and data collection
• Support budget monitoring and grant deliverables
• Partner with community organizations to expand access to healthcare services

Community Outreach

• Develop and maintain relationships with community agencies, schools, and service organizations
• Represent the clinic at community events, health fairs, and outreach initiatives
• Support programs designed to increase healthcare access for underserved populations

Qualifications

• Master of Science in Nursing (MSN) required
• Doctoral degree in Nursing, Healthcare, or a related field preferred

Licensure & Certification

• Family Nurse Practitioner (FNP) certification required
• Current and unencumbered Advanced Practice Registered Nurse (APRN) certification and Registered Nurse (RN) licensure
• Active California furnishing number
• Active National Provider Identifier (NPI)
• Active DEA registration
• Current national and state board certification as a Nurse Practitioner through an approved organization such as ANCC, AANP, or NCC

Experience

• Minimum of 5 years of healthcare leadership or management experience
• Pediatric clinical experience required, including the ability to provide care in elementary school settings
• Experience providing care across the lifespan
• Experience serving underserved, vulnerable, or historically marginalized populations
• Experience managing healthcare programs, clinical teams, or community-based services
• Experience working with populations experiencing homelessness strongly preferred
• Grant-funded healthcare program experience preferred

Preferred Qualifications

• Federal grant management experience
• Nurse faculty or preceptor experience
• Experience partnering with community-based organizations and public health initiatives
• Experience supporting behavioral health and primary care integration

Location & Travel Requirements

• Must reside in San Diego, CA
• Ability and willingness to travel up to 75%
• Frequent travel between clinic and community sites is required
• Travel to Los Angeles will be required as needed
• Travel expenses are reimbursed
