Job Summary

We are seeking compassionate, dependable, and experienced Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our nursing team.

The LPN/RN is responsible for providing high-quality nursing care to residents in accordance with physician orders, individualized care plans, facility policies, and applicable state and federal regulations. The ideal candidate demonstrates strong clinical judgment, leadership, professionalism, and a commitment to resident safety and quality of care.

Essential Duties and Responsibilities

• Provide direct nursing care to residents in accordance with physician orders and established plans of care.

• Administer medications and treatments accurately and timely.

• Perform resident assessments and identify changes in condition.

• Immediately notify the physician, supervisor, and responsible party of significant changes in a resident's condition as required.

• Monitor residents following falls, incidents, hospital returns, new medications, treatments, and changes in condition.

• Complete required nursing documentation accurately, thoroughly, and within established timeframes.

• Maintain accurate Medication Administration Records (MARs) and Treatment Administration Records (TARs).

• Obtain and document vital signs, blood glucose readings, weights, intake/output, and other clinical information as ordered.

• Provide wound care and other skilled nursing treatments within the nurse's scope of practice.

• Monitor and manage IV therapy, oxygen therapy, enteral feeding, catheter care, ostomy care, and other clinical services as applicable.

• Follow infection prevention and control practices, including appropriate isolation precautions and use of personal protective equipment.

• Participate in fall prevention, skin integrity, weight management, behavior monitoring, infection control, and other facility clinical programs.

• Ensure physician orders are accurately transcribed, implemented, and followed.

• Communicate with physicians, nurse practitioners, pharmacy, laboratory, therapy, hospice, and other healthcare providers as necessary.

• Review laboratory results and diagnostic reports and ensure appropriate physician notification and follow-up.

• Provide supervision and direction to Certified Nursing Assistants (CNAs) assigned to the unit.

• Ensure CNAs complete assigned resident care and promptly address identified concerns.

• Make regular resident rounds and ensure call lights and resident needs are addressed timely.

• Provide appropriate interventions during emergency situations.

• Complete incident reports and participate in investigations and root-cause reviews when required.

• Participate in admissions, discharges, transfers, and hospital returns.

• Complete admission assessments and required documentation.

• Ensure residents receive medications, treatments, nutrition, hydration, ADL assistance, repositioning, and other services according to their care plans.

• Maintain resident dignity, privacy, confidentiality, and rights at all times.

• Immediately report suspected abuse, neglect, exploitation, mistreatment, injuries of unknown origin, or other reportable concerns according to facility policy.

• Communicate professionally with residents, families, coworkers, physicians, and members of the interdisciplinary team.

• Participate in shift-to-shift nursing reports, clinical meetings, staff meetings, in-services, and mandatory education.

• Maintain a clean, organized, and safe medication cart and nursing station.

• Follow all facility policies and procedures and applicable IDPH, CMS, and professional nursing standards.

• Perform other nursing duties as assigned by the Director of Nursing, Assistant Director of Nursing, Nursing Supervisor, or Administrator.

Charge Nurse Responsibilities

When assigned as Charge Nurse, the LPN/RN will:

• Provide leadership and clinical oversight for the assigned nursing unit.

• Supervise CNAs and other assigned nursing personnel.

• Assign resident care responsibilities appropriately.

• Ensure nursing assignments are completed during the shift.

• Conduct rounds throughout the unit.

• Address resident, family, and staff concerns promptly and professionally.

• Notify nursing leadership of significant clinical, staffing, safety, or operational concerns.

• Ensure proper completion of required documentation before the end of the shift.

• Provide a thorough report to the oncoming nurse.

Qualifications

LPN

• Current and unrestricted Illinois Licensed Practical Nurse (LPN) license.

• Graduate of an accredited Practical Nursing program.

• Current CPR/BLS certification preferred.

• Previous skilled nursing, rehabilitation, long-term care, or similar healthcare experience preferred.

RN

• Current and unrestricted Illinois Registered Nurse (RN) license.

• Graduate of an accredited Registered Nursing program.

• Current CPR/BLS certification preferred.

• Previous skilled nursing, rehabilitation, long-term care, or similar healthcare experience preferred.

• Charge Nurse or supervisory experience preferred.

Physical Requirements

The position requires the ability to:

• Stand and walk for extended periods.

• Bend, stoop, reach, push, and pull.

• Assist with resident transfers and repositioning when necessary.

• Safely operate mechanical lifts and other resident-care equipment.

• Respond promptly to emergency situations throughout the facility.
