REQUIREMENTS * Must have a Bachelor’s degree in a Human Services related field. * One year of experience in Case Management, preferably in Substance Use Disorder. * Experience working with diverse populations. * Experienced in Motivational Interviewing techniques preferred. * Experience conducting needs assessments. * Knowledge of Hampton Roads community resources and social service providers is preferred. * Lived experience in recovery from substance use disorder is preferred * This position requires driving your personal vehicle for company business. Must be privileged to drive according to the criteria set forth by Agency guidelines. + Valid driver’s license in the jurisdiction of residence + Vehicle liability coverage (on occasion, may be required to transport clients in personal vehicle) * Acceptable Criminal background check, OIG Exclusion check * Negative Drug Screening * CPR/First Aid Certification must be acquired and maintained once employed PHYSICAL REQUIREMENTS The physical requirements described below are representative of those that must be met by an employee to successfully perform the essential duties of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential duties. * The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary. * The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment. * Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment. * Operating office equipment requiring continuous or repetitive hand/arm movements. * The ability to remain in a sitting position for extended periods of time.
Description The Building Bridges to Hope (BBtH) Prevention Specialist serves as the initial point of contact for referrals and access to resources and prevention services for individuals with Substance Use Disorder (SUD) for Volunteers of America Chesapeake and Carolina’s (VOACC) SPF-PFS Program. This role focuses on prevention efforts and relapse prevention, providing coordinated, individualized support to clients to address their specific needs and help them achieve their goals. The Prevention Specialist will utilize a comprehensive approach to link clients with appropriate services and support. Responsibilities: * Serve as the initial point of contact for referrals and access to resources and treatment for individuals with SUD. * Provide individualized case management services to clients, including assessment, goal-setting, and ongoing support. * Coordinate and facilitate referrals to services for therapy, groups, medication management, Medication-Assisted Treatment (MAT), and other Social Determinants of Health (SDOH) as needed. * Organize individual and small psychoeducation groups, both in-person and virtual, including mental health promotion services, in accordance with SPF-PFS Program Grant Requirements. * Utilize Motivational Interviewing techniques to support clients in their recovery journey and prevent relapse. * Conduct screenings and assessments, such as the Patient Health Questionnaire-9 (PHQ-9) and the Columbia-Suicide Severity Rating Scale (C-SSRS), as needed to provide appropriate referrals and support. * Maintain accurate and up-to-date client records, including progress notes and treatment plans. * Collaborate with internal and external stakeholders, including community organizations and treatment providers, to ensure comprehensive care for clients. * Maintain CPR/First Aid Certification and respond effectively in crisis situations. * Demonstrate cultural competency and sensitivity in working with diverse populations, including youth, LGBTQ+ individuals, pregnant women, military/veterans, and individuals in recovery. * Participate in ongoing training and professional development
