Description: Med Tech Type: Full-Time and Part-Time/On-Call (PRN) available Shift/Schedule: * FT: Day shift (6am-2pm), 4 days on/2 days off/rotating schedule * FT: Swing shift (2pm-10pm), 4 days on/2 days off/rotating schedule * PRN: On-Call, All shifts available, Weekend availability needed Wage: $22.47-$27.97/hr DOE Location: Avamere at Park Place - 8445 SW Hemlock St., Portland, OR 97223 - Near Tigard, OR Apply at www.TeamAvamere.com Job Summary The primary role of the Medication Tech is to administer medications and provide direct care to residents. Responsibilities: * Pass medications according to physician orders, delegated by and under the supervision of a licensed nurse and in accordance with facility policy. * Administer accurately and timely. * Record medications administered in the resident's record. * Notify the pharmacy (or family when applicable) when medications need to be refilled. * Maintain a clean and organized medication cart and medication room. * Monitor and report any/all side effects of medications to the Director of Health Services. * Obtain/provide shift report.
Record sufficient information related to medication administration to implement accurate changes to medication orders. * Follow comprehensive care plans related to medication administration based on each resident's needs and their comprehensive assessment as assigned. * Assisting with medical appointments, communicating with healthcare providers, and advocating for the residents' needs * Observing residents for changes in health, reporting concerns to medical professionals, and monitoring vital signs. * Document information pertaining to changes in residents' status, personal needs, and communications with outside providers in the electronic medical record. * Complete accident and incident reports in a timely manner and according to facility policy * Perform delegated nursing services as assigned by the RN. * Participate in in-service education programs as assigned. * Promptly report changes in resident conditions to the Director of Health Services. * Follow written and oral directions. * Perform all duties assigned in an effective, cooperative, timely and professional manner. * Be responsible for the safety of residents under their supervision. * Work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies, and ancillary service providers. * Assist new employees in following established facility policies and procedures. * Maintain confidentiality of resident and facility information. * Perform other duties as assigned by the manager or supervisor. Requirements: * Experience as a Caregiver and Medication Tech in a Memory Care Unit and or Assisted Living Facility. * Must have the ability to obtain and maintain active CPR/First Aid Certification during employment. * Must have the ability to obtain a Food Handler's Card. * Experience in electronic medical records documentation. * Must be 18 years or older. * Friendly, attentive, and professional demeanor with residents and guests. * A genuine desire to improve the quality of life for seniors. * Create and uphold an atmosphere of warmth, patience, enthusiasm, and a calm and cheerful environment.
