Site: This position travels to participant's home and involves extensive use of reliable transportation throughout San Diego County for home visits. Job Summary: Under the direction of the In-Home Services Director, provides skilled nursing services (medication administration, wound care, etc.) to the participants of St. Paul’s PACE in their homes. Maintains the participants’ medical record with thorough documentation. Communicates participants’ changes and progress to appropriate staff. Qualifications and Requirements: * Education - Graduate of a school of Licensed Vocational nursing. Current California Licensed Vocational Nurse license, CPR and first aid certification required. Shall be at least 18 years of age. * Experience – A minimum of one year’s experience in working with the frail or elderly population required.
