As a CNA Home Health Aid, you will assist clients with daily living activities, such as bathing, dressing, and grooming, while promoting their independence and well-being. You will work closely with our team of healthcare professionals to develop and implement personalized care plans that meet each client's unique needs. Responsibilities:

Provide personal care assistance to clients, including bathing, dressing, and grooming Assist with activities of daily living (ADLs) such as toileting, transferring, and ambulating Assist with meal preparation and consumption Maintain a clean and safe living environment Develop and implement personalized care plans, in collaboration with our healthcare team Monitor and report changes in client condition to our healthcare team Provide emotional support and companionship to clients
