Help Youth and Families Move from Referral to Meaningful Services
EmotiCare is seeking a compassionate, organized, and persistent Intake & Client Engagement Coordinator to serve as the primary liaison between clients, families, referral sources, and EmotiCare programs.
This position combines intake coordination, community-based outreach, and case management. The coordinator does more than complete an intake—the coordinator remains involved until the client is successfully connected with services and actively participating.
The primary goal of this position is to prevent clients from becoming lost or disengaged after intake. Through consistent telephone, telehealth, and electronic outreach, the coordinator will maintain contact with clients and families, identify barriers, coordinate next steps, and help services begin as quickly as possible.
Key Responsibilities
Receive and screen referrals for eligibility and service appropriateness
Gather required demographic, clinical, insurance, and supporting documentation
Coordinate intake appointments, assessments, admissions, and program onboarding
Serve as the primary liaison between the client or family and the receiving EmotiCare program
Maintain active follow-up after intake until services have started and the client is successfully engaged
Contact clients and families by telephone, telehealth, text, email, and other approved communication methods
Provide reminders, encouragement, and practical support to promote continued participation
Track each client’s progress from initial referral through admission and connection to services
Identify and help resolve barriers involving scheduling, transportation, documentation, communication, or family readiness
Alert program leadership when a client is at risk of experiencing a delay or disengaging from services
Coordinate warm handoffs to therapists, community-based outreach workers, case managers, and other assigned providers
Communicate with guardians, referral sources, health plans, case managers, and other involved professionals
Coordinate appointments, assessments, transportation needs, and other service-related activities
Provide telehealth and community-based support to clients and families as assigned
Help clients and families connect with appropriate internal and community resources
Participate in treatment planning, case staffing, service coordination, and discharge planning
Coordinate discharge activities, aftercare recommendations, and continuity of care
Maintain accurate referral, outreach, engagement, and case coordination records
Complete timely and professional documentation of all client contacts and follow-up efforts
Protect confidential information and comply with HIPAA, organizational policies, and applicable behavioral health requirements
