Licensed Practical Nurse (LPN) or Registered Nurse (RN) Coordinator (MDS)
Job Summary:
We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) Coordinator (MDS) to join our team at Blue Palms Health and Rehabilitation Center at Fletcher in Tampa, FL, USA. As a key member of our healthcare team, you will be responsible for coordinating the care of our patients, ensuring that their physical, emotional, and social needs are met. Responsibilities:
Coordinate patient care by developing, implementing, and evaluating individualized care plans in collaboration with the interdisciplinary team
Conduct thorough assessments of patients' physical and emotional needs, identifying potential risks and developing interventions to mitigate them
Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions and continuity of care
Participate in interdisciplinary team meetings to discuss patient progress, address concerns, and develop strategies for improved outcomes
Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and progress notes
Collaborate with healthcare professionals to provide high-quality, patient-centered care that meets the standards of our organization
Requirements:
Current LPN or RN license in the state of Florida
Minimum 1-2 years of experience in a long-term care or rehabilitation setting
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment with multiple priorities and deadlines
Basic computer skills and familiarity with electronic medical records
